How to submit file attachment questionnaires via app

1. Navigate to your Questionnaire Concierge Dashboard > Click “New Questionnaire” 

2. Fill out the “New Questionnaire” form as shown below:

3. For Expedited Requests, checkmark the “Expedited” button
